How to schedule your appointment quick and easy

• Submit an appointment request form online or call our office at 254-239-0805. For online requests, one of our team members will review your submission and reach out to you soon to confirm your appointment.
• Fill out your paperwork online. This will save you time when you come in for your appointment.
  
• Arrive a few minutes early in case we have a few follow-up questions for you. You can also complete your paperwork in person if you weren’t able to do it online.
